Care Management Services

The following agencies and individuals provide assistance and guidance in coordinating long-term care, in-home care, or other services based on a needs assessment and an individualized care plan.

American Elder Advocates -744-4526. AEA provides services in four areas: Care Management, Elder Advocacy, Consultation, and Mediation. AEA is also able to manage local services for long distance care and to provide support for family caregivers. The company offers experience and expertise to care for the elder in distress with dignity.

Care Coordination McKee Medical Center - 635-4138. Care Coordination is a free service provided by McKee Medical Center nurses and social workers for area residents considered "high risk" due to complex medical problems and/or health care issues resulting in frequent emergency room visits or hospital admissions. Care Coordination is designed to assist persons in achieving and maintaining their highest level of wellness, to help them make appropriate use of health care settings and medical services, and to link persons with community resources and providers.

Catholic Charities Northern (CCN) Elderly Services — Ft. Collins: 484-5010, Loveland: 663-1880. Volunteer Programs: Caring Hearts — Volunteers provide respite care, companionship and practical assistance to low-income individuals and families in Larimer County. Friendly Visitors — Volunteers provide friendship and companionship to lonely seniors or individuals with disabilities. If you are in need of caregiver relief or wish to volunteer, call 484-5010.

Community Case Management — Poudre Valley Health System, Poudre Valley Hospital: 495-8554. Poudre Valley Hospital's Community Case Management Program seeks to help high-risk individuals who lack adequate resources, are chronically or terminally ill, and/or are not eligible for other in-home services. Case managers work closely with clients, family caregivers, health care providers, and community agencies. They provide comprehensive in-home assessments and treatment planning, information and referral, and individual and family education and counseling. There is no charge for PVHS Community Case Management services.

Consultants for Aging Families — Nancy Driskill: 498-0730. Consultants for Aging Families are experts in family problem-solving with regard to communication, parent care, and alternative housing. They offer counseling for elder care planning, guardianships, and health care proxies, utilizing community-wide information and referral resources. Consultants for Aging Families also conducts private home care provider searches, screening, and hiring, with follow-up supervision and management. (This link will open a new browser window. Family Caregiver Support Program -- Office on Aging: 970-498-7758. The Caregiver Support Program is a direct service of the Larimer County Office on Aging. It is a free community service to assist family caregivers in finding the answers they need. A caregiver coordinator will help the family caregiver assess their caregiving situation, create an individualized plan, connect with appropriate community resources, and provide needed support. This program offers a $300 respite grant to help pay for the most appropriate respite care services. There is no charge for consultation and services.

Legacy Services, LLC--970672-0888. Legacy Services, LLC is an independent, professional, certified care management organization created to provide older adults and their families’ peace of mind and consultation to provide the support and guidance needed to optimize caring for an aging loved one. Legacy is owned and operated by Becky Lindsey. Becky has over 10 years experience in medical social work and counseling. Becky is a Licensed Clinical Social Worker and a Certified Geriatric Care Manager. She is a member of the National Association of Social Workers (NASW), the National Association of Professional Geriatric Care Managers (NAPGCM)

Money Matters LLC - 970-484-8208.  Jenny Illig, a member of the American Association of Daily Money Managers, provides trustworthy and time-saving daily money management services. Her clients value the personal attention they receive while Jenny works with them to make sure bills are paid on time (avoiding late fees or overpayments), make deposits, and reconcile bank statements. She also helps her clients develop and manage a budget. Jenny excels at organizing and tracking financial and medical insurance records, even helping clients with insurance claim and billing disputes. She can help clients manage estate planning by working with the client's attorneys, financial planners and accountants. Jenny has a MBA degree from Colorado State University and more than 25 years of experience in bookkeeping and recordkeeping.

Peggy Haller, MA — Gerontologist: 493-0895.

Seniors in Transition, LLC — Kirsten Hartman: 204-6977. Seniors in Transition provides case management to help people create a confident and secure lifestyle. Facilitating decision-making, assisting with details of a move, and referrals to other community providers are the critical support services that clients appreciate. Additional services include daily money management such as bill paying, check book balancing, and filing of insurance claims. Kirsten is experienced in guardianship, conservatorship, real estate, and retirement communities. Visitation services can be arranged as needed. (This link will open a new browser window. Lone Pine Estate Services — Jo Inman: 224-2637. Lone Pine Estate Services provides a wide range of estate-related services to seniors and their families. These services cover estate liquidation including sorting, pricing, and conduction of sale. We offer case management for seniors and their families helping them through the process of selling the family home, such as finding housing, setting up repairs for the sale of the house, and liquidation of non-titled property.

Larimer County Options for Long Term-Care498-7780. Options for Long Term Care provides access to publicly funded programs (Medicaid), that provide support to older persons and people with disabilities. These programs are designed to help people continue to llive in their own homes or in other community settings. To qualify, a person must meet both functional and financial requirements.
